This "TOP FAN" sponsorship form is for individual supporters and family who wish to donate funds to this 14U Rep team to help pay for costs that are incurred throughout the season such as indoor training, team equipment etc. In return, the team will add your name to the bottom of their banner that they travel with to games & tournaments throughout the Niagara area and across Ontario.
